Medical Evaluation Form

Cornell University's Animal Users Health and Safety Program (AUHSP) covers all persons having direct or indirect contact with animals for the purpose of evaluating the human-health risks associated with that contact. A risk assessment is conducted for all individuals to ensure that health risks are managed to an acceptable level. For many, a crucial part of the risk assessment involves completing an annual Medical Evaluation Form (MEF).

In order to protect the health and safety of individuals working with research animals, the AUHSP requires that those involved in the medical surveillance program, and any person named to moderate or high-risk protocols or identified as having a medical condition that may affect their personal risk level, complete the MEF. Failure to submit a required MEF will impact an individual's ability to access Cornell's animal care facilities.

Instructions for completing your MEF

Completing your MEF takes approximately 10 minutes. Once you have submitted your MEF, an Occupational Medicine staff member will contact you if there is a need for further information or a medical appointment.

Individuals with a Cornell NetID:

  1. Log in to myCority, our secure patient portal, using your NetID and password.
  2. Click the Questionnaire icon.
  3. Select the questionnaire, and answer all questions. (You may select “Save” if you need to stop and finish later.)
  4. Select Submit at the top of the page to submit your questionnaire. You will see a “Success – Record submitted” confirmation. 
  5. Log out of myCority or close your browser to protect your personal information. 

Individuals without a Cornell NetID:

  1. Click on the link that you receive in your email from Occupational Medicine. 
  2. Select Applicant. 
  3. Select Next. 
  4. Enter your Temporary ID from the email you received.  
  5. Enter your Last Name and First Name.  
  6. Select Next. 
  7. Answer all questions in the questionnaire. At any point, you may select “Save” to save and return to your answers.
  8. Select Submit at the top of the page to submit your questionnaire. You will see a “Success – Record submitted” confirmation. 
  9. Log out or close your browser to protect your personal information.
